Camper Forms
All of our Camper Forms must be accessed through your online account. Every camper, once enrolled, has an online account, whether or not you enrolled online.
To access your online account, please simply click the 'My Account' link. After you enter your login information, click on 'Forms and Documents' under the Forms Dashboard and there you will see which forms are needed for your camper and when they are due.
The required forms for Day Camp are:
- Health History (online form)
- Parent Consent and Insurance - include a copy of front and back of health insurance cards (print and fax)
The required forms for Resident Camp are:
- Camper Personal History (online form)
- Health History (online form)
- Parent Consent and Insurance - include a copy of front and back of health insurance cards (print and fax)
- Doctors Form (print and fax)
- Immunization Form (online and print and fax)
Additional forms for optional activities can also be found for:
- Swim Lessons
- Horseback Riding Lessons
- Alternate Adult Pick-up - if applicable
- Out of Town Travel Form
*All of the forms are due two (2) weeks prior to the start of your camper's session.
